Denton County Public Health & City of Denton | Texas
In November 2023, Denton County and the City of Denton jointly introduced FindHelp Denton County, an online resource directory powered by Findhelp that centralizes free or reduced-cost services for local residents. This intentional city-county partnership has established FindHelp Denton as a unified, accessible digital hub, empowering residents to quickly find and connect to vital assistance within their community.

Funding a collaborative, intentional resource hub implementation
Recognized with a 2025 Model Practice Award by the National Association of County and City Health Officials (NACCHO) for their innovative approach, Denton County led a presentation at the 2025 Connect Summit Conference that outlined best practices for local governments to fund and implement digital resource platforms.
- The City of Denton prominently features the platform under its “Community Resources” and “Homelessness Solutions” webpages, highlighting tools like Coordinated Entry, food resources, and mental health navigation.
- The platform is integrated into key city and county initiatives, offering direct links for residents to contact service navigators or complete referral forms.
- The result is a no-wrong door system that reduces resident confusion and saves staff time previously spent redirecting phone calls and emails.
Howard County Health Department | Maryland
Since 2021 CAREAPP (powered by Findhelp) has made it easy for Howard County residents to find and connect with over 2,500 programs to address their needs. Through effective CBO partnerships and community engagement coordination, Howard County has achieved an 86% referral response rate–this means that nearly 9 out of 10 referrals have been updated, showing a high level of community engagement with the platform.

This high level of engagement gives the county visibility into the referral lifecycle, drastically reducing the administrative burden of follow-up and confirming that partner CBOs are actively connecting with residents.
Resource management “superpowers”
The County uses Findhelp data and infrastructure, via the Program Manager tool, to help expand and maintain their local resource network. County staff regularly use the tool to both streamline internal Health Department program updates and provide real-time technical assistance to partner CBOs.

Boulder County | Colorado
Find Help Boulder County is a collaboration between Boulder County Public Health, Human Services, and Early Childhood Council; the City of Boulder; and a dozen local CBOs. The platform aligns with the county’s mission to address social, economic, and environmental conditions to ensure that all residents have the opportunity for a healthy life.

Community-led implementation
Boulder County began implementing their Findhelp platform in late 2024:
- They county collaborated with community partners who voted for Findhelp over other options.
- As part of the implementation, Findhelp migrated county data from one platform to another to ensure continuity of care.
- Even as the county ramps up their community engagement work, they’ve already achieved a 61% closed-loop referral rate.
- This means that for over 6 in 10 referrals, the county has confirmed whether or not the resident’s need was actually met—moving beyond tracking activity to measuring tangible outcomes.
- This level of data and infrastructure is critical for stakeholder engagement and strategic planning.

Porter County Health Department | Indiana
Find Help Porter County is a collaboration between the county Health Department and Sheriff’s Office, Northwest Health, CoAction, and Valparaiso University. With feedback from the community, Porter County customized their Findhelp platform to residents’ needs, featuring navigation categories like Addiction & Recovery, Mental Health & Support, Families & Children, and Community Connection.

Fast, targeted support for critical populations
To support important initiatives like reentry, aging in place, and economic stability, Porter County has effectively harnessed Findhelp’s Favorite Folders functionality to organize vital programs into intuitive categories, directly from their Findhelp homepage:
