Arkansas Children’s Hospital (ACH)
A Findhelp customer since 2020, ACH’s success is due to incorporating their social care referrals and community engagement efforts into larger SDoH strategies, known as the First 100 Days Social Risk Factors.
- ACH’s Community Benefits department regularly reaches out to CBOs, helps them claim their listings, and hosts training sessions or promotes Findhelp’s training options.
- Their Trusted Network of CBO partners has a 75% referral response rate.
Cedars-Sinai
Cedars-Sinai and Findhelp have been working together since April 2021 to improve patient and community health and develop a cross-departmental approach to connecting patients to supportive services.
- Cedars-Sinai regularly leads training sessions for community organizations in the Los Angeles region.
- They provide information, education, and one-on-one support for utilizing Findhelp’s free client management tools

Eskenazai Health
A Findhelp customer since 2020, Eskenazi uses a Findhelp-Epic integration to send more than 1,000 referrals a month to local CBO and FQHC partners.
- Eskenazi employs dedicated ‘community weavers’, a blend of patient navigator and community liaison.
- Community weavers identify residents’ SDoH needs (in Epic), build relationships with CBOs, and send them referrals (via Findhelp).
- They are piloting a new program through their After Visit Summary in Epic that provides vouchers to local food programs
- They continue to invest in programs like a traveling food pantry in a refurbished school bus
