An interoperable social care platform
Findhelp is interoperable, so you can avoid using multiple systems and duplicate data entry. We connect to other systems so you can have the information you need to provide seamless, whole-person care using the system you prefer.
Smooth data exchange to connect partners
Findhelp connects to different systems and applications so you can provide care without jumping to multiple platforms. Even with seamless data sharing, we never compromise privacy.
System integrations
We strategically partner with a growing number of EHRs, care management, and intake tools, like Epic, Cerner, eClinicalWorks, and Salesforce.
Interoperable framework
Never miss a referral when you use our APIs and webhooks. These capabilities enable you to receive referrals from any system, and to review and respond using your preferred platform.
Database connections
We display eligibility information for the person you are helping right inside Findhelp. We do this with API connections to Medicaid Member Information Systems and health plan member databases.
Health information exchanges
Give staff access to health and social care information using our bidirectional data exchange capabilities. We plug right into regional and statewide health information exchanges to create a longitudinal care record and improve whole-person care.
Built-in data standards
We automatically encode social care screening tools and social care services so data can be aggregated and shared. We use the most up-to-date version of the United States Core Data for Interoperability standards for social determinants of health.Community Partnerships
